“Companies need centralized knowledge bases, strong data governance and ongoing stakeholder collaboration to keep AI accurate and secure,” Phan says. “When I led the build of a retrieval-augmented generation (RAG) system for RFP automation, success came from creating a unified repository across Legal, Security, HR, Product, Finance and Marketing.”

“Organizations should use strong knowledge-management practices, such as maintaining well-structured, versioned knowledge bases and continuously refreshing data pipelines,” Nuthalapati says.
He also points to RAG as a practical safeguard to ensure AI pulls from the latest approved sources.
“Context management, access controls, data governance and audit trails ensure relevance, prevent drift and protect sensitive information,” he says.
Treat Knowledge as a Living, Governed Ecosystem
Raghu Para of Ford Motor Company argues that AI knowledge management must take a holistic approach.
“Companies need a centralized repository with a clear owner responsible for updates and quality,” Para says.
He also stresses the importance of automated versioning and change-tracking; continuous validation through expert review and feedback loops for catching errors and dated content; role-based access, encryption and data anonymization; and a taxonomy and metadata system for context-aware retrieval and reasoning across documents.
“Together,” Para says, “these measures create a living, governed knowledge ecosystem that ensures AI assistants remain reliable, current and resilient at scale.”
Human Stewardship Is the Missing Operating Model
According to Gordon Pelosse, Executive Vice President of Partnerships and Enterprise Strategy at AiCerts, the biggest misconception in enterprise AI is that knowledge management is primarily a technology problem: “The bottleneck isn’t the model—it’s the quality, stewardship and governance of the underlying knowledge,” Pelosse says.
He emphasizes human accountability. “Domain experts serve as stewards, validating and approving updates; employees provide corrections and new data,” he says. “Central governance and human reviews for errors and model drift provide control and consistency.”
Pelosse also highlights the cultural dimension. “Worker training and accountability make knowledge a managed asset rather than a governed silo,” he says. “Human expertise, governance and accountability are what keep knowledge accurate, up to date and secure.”

Evaluation and Calibration Matter More Than Retrieval Alone
Sinan Ozdemir, AI thought leader and Founder of Crucible, challenges the assumption that RAG alone solves enterprise knowledge problems, citing it as a workaround rather than a solution: “You’re asking a model that doesn’t understand your business to retrieve documents it can’t fully interpret.”
He advocates encoding institutional knowledge directly into models. “Fine-tuning your embedders and re-rankers can dramatically improve retrieval quality,” he says, but notes that even that’s not enough without evaluation.
“Build the evaluation pipeline first,” Ozdemir says. “Measure calibration, not just accuracy. Then you’ll know which knowledge gaps to fix, potentially with fine-tuning.”
Culture Determines Whether Knowledge Stays Alive
For Divya Parekh, Founder of executive coaching brand DivyaParekh.com, the quality of AI knowledge reflects organizational culture as much as process. “The knowledge used to train an AI assistant determines its quality,” Parekh says. “The way we manage knowledge makes or breaks it.”
She cautions against over-collecting data. “Most organizations don’t need bigger databases—they need knowledge that stays alive,” she says. “That looks like regular clean-ups, clear owners who keep information current, version control that avoids confusion and simple governance that prevents outdated guidance from slipping into daily decisions.”
Psychological safety is critical, Parekh adds.
“If people don’t feel safe speaking up when something is wrong, AI slowly drifts out of reality,” she says. “Accuracy depends on honesty.”
Canonical Sources and Ongoing Audits Build Trust
Jason Barnard, Founder and CEO of Kalicube, frames internal AI as an educational system. “Internal AI is a strategic asset, but its value hinges entirely on the quality of its knowledge,” Barnard says. “Think of this as algorithmic education: teaching your machine a clear, consistent curriculum.”
He outlines three steps: Establish a canonical source of truth (by designating a single knowledge base as the primary factual source), engineer a corroboration ecosystem (by ensuring all documents and systems link back to this source) and implement ongoing audits (by regularly verifying accuracy, relevance and consistency).
He adds, “This approach builds algorithmic stability and helps your AI move from being a simple tool to a trusted partner.”
Information Hygiene Determines AI Outcomes
Entrepreneur, investor, advisor and enterprise AI strategist Jim Liddle urges leaders to treat knowledge bases like curated libraries. “If you turn them into dumping grounds, you get garbage-in, garbage-out at scale,” Liddle says.
He advocates integration with source systems like HR platforms or Slack for real-time syncing, as well as content expiration dates and automated review triggers.
“Stale knowledge is out-of-date knowledge,” Liddle says.
Security, he adds, also requires visibility: “Ensure you log and audit AI queries to detect unusual access patterns or potential data exfiltration. The AI will only be as good as the information hygiene that is applied.”

At Texas Health, Mohan Krishna Mannava, Data Analytics Leader, champions what he calls “Knowledge Sovereignty,” where data is treated as a dynamic, self-checking system.
The first step, he says, is a decentralized verification mesh: “A secondary AI model must continuously cross-check the main agent’s outputs against data sources, assigning a trust score to every fact. This ensures accuracy and security.”
Along with expiration dates for every piece of data, zero-trust principles complete the picture.
“Segment all proprietary data,” Mannava says. “The AI must be individually authorized for every single query, securing your specialized knowledge and preventing misuse.”

For Sathish Anumula, Enterprise and Business Architect at IBM Corporation, KnowledgeOps is essential.
“Use RAG to get the best outcomes. This architecture stops the AI from making things up and forces it to use real ‘ground truth’ documents,” he says.

“The AI must do what the user says it can do,” Anumula says. “For instance, the AI shouldn’t get a file if an employee can’t see it directly.” This, alongside clearing out PII before indexing, ensures private information stays safe.

He adds, “Human oversight must be elevated from an exception handler to an embedded function within the content lifecycle.”
Prevent Context Collision and Trust Decay
Bhubalan Mani, Lead of Supply Chain Technology and Analytics at GARMIN, warns that static knowledge bases create context collision.
“The real challenge is context collision, where contradictory documents inform the same response,” Mani says.
He advocates for Knowledge Graph RAG—mapping relationships explicitly to achieve 90% accuracy versus 50% for vector search.
Security, Mani adds, requires freshness verification.
“Build automated staleness detection with verification loops where secondary models cross-check outputs. Knowledge that cannot prove freshness becomes liability,” he says.

“The biggest risk isn’t AI hallucination; it’s institutional amnesia codified by outdated data,” says Pradeep Kumar Muthukamatchi, Principal Cloud Architect at Microsoft. “We must shift from simply ‘storing files’ to ‘curating intelligence.’”
He calls for a Continuous-Integration approach, where every fact is treated like code that requires testing before being deployed, and automated expiration dates for data.
“For security, move beyond access lists to ‘contextual masking,’” he adds, “ensuring the AI understands ‘who’ is asking before it even retrieves the ‘what.’”
